Nice list. It does not however suggest the root causes for these costs, only where they show up in the process.
The vast majority of organizations I helped get to on time, on schedule and good quality delivery had problems in their commitment process – primarily in realistically estimating the effort.
Once we got over the cultural inhibitions to making good estimates (which usually meant longer schedules for example) not only did they start meeting their schedules, but the cost – especially due to fixing defects – dropped dramatically.
It is amazing how many problems decrease in intensity, when you just allow a team a realistic plan for getting the job done.
